School Health Requirements

For the registration process, the following documents are required:

• Immunization/shot record; required by RI State Law: R.I. Immunization Requirements

All students entering from out of state are required to provide documentation of a physical exam (done up to 12 months before or 6 months after school entry)

Proof of Immunizations must be provided before registration can be complete.  Students will not be allowed to attend Rogers High School until this documentation has been provided.   *Immunizations may be obtained free of charge at RHS with the Vaccinate Before You Graduate Program-contact the Health Office for information
