Use this form for immunizations your child will not be receiving because a parent has declined the immunization. Your child will be excluded from school should there be an outbreak of the disease covered by the exempted immunization.

RI Temporary Immunization Exemption Certificate

Use this form for immunizations your child will receive at a scheduled doctor appointment. Must include documentation of the upcoming appointment from the doctor's office.
